Please note every time I attempt to retake the quiz I am prompted quiz completed. I seeing 25 questions but was only given 12 questions.

I experienced the same problem you can do 1 of two things. Depending on the browser you are using clear your browsing history, which clears your cache. That works or you can use another device and do the quiz.
To clear your browsing history click on the three dots to the top right of your browser, click on setting and then click on clear browsing history.
